This symposium examines how understanding OSA endotypes and phenotypes can guide tailored therapy beyond CPAP. Drawing on current evidence, it explores the physiological traits contributing to OSA upper-airway anatomy, muscle responsiveness, arousal threshold, loop gain, and ventilatory control, and their therapeutic implications. Emerging phenotype-targeted interventions will be reviewed, including mandibular advancement, positional therapy, hypoglossal nerve stimulation, and pharmacological agents. The session will also discuss biomarker development and the use of novel technologies to integrate endo-phenotyping into clinical care.
